In ancient times, eye care consisted of salves made from metals and animal brains and rituals involving chanted prayers (Eye Doctors Through Time, 2025). Now, thanks to innovations in ophthalmology, eye care looks more like pain-free, 30-minute surgeries. Since the first record of eye surgery in the Code of Hammurabi, circa 2250 BCE, there have been many incredible innovations in eye care. However, here are a few of the greatest modern innovations to eye surgery: intraocular lenses (IOLs), Small Incision Lenticule Extraction (SMILE), and AI-powered diagnostics.
First, IOLs are a major innovation in cataract surgery–the most common type of eye surgery worldwide. IOLs improve post-cataract surgery vision and often eliminate the need for eyeglasses by replacing the cataract-affected natural lens of the eye to restore clear vision. Traditionally, IOLs were monofocal, meaning that they only provided clear vision at a fixed distance. However, new IOL technology provides custom vision correction options like multifocal IOLs, accommodative IOLs, toric IOLs, and light-adjustable IOLs (Innovations in Vision Correction Surgery, 2023). Fortunately, the implantation of IOLs does not take long–up to 15-30 minutes (Eye Lens Replacement, 2023). In conclusion, the invention of IOLs contributes greatly to modern ophthalmology and, in particular, the treatment of cataracts.
Second, SMILE is a great example of an innovation that takes an existing product and makes it better. SMILE is an alternative to the popular LASIK eye surgery, characterized by its flapless surgical procedure: whereas the opening caused by LASIK is approximately 20 millimetres, the opening caused by SMILE is a mere 4-6 millimetres (SMILE Eye Surgery vs. LASIK: A Detailed Comparison, 2021), preventing the formation of a corneal flap, which can sometimes cause flap-related complications and slower recovery (Innovations in Vision Correction Surgery, 2023). The operation includes the use of a femtosecond laser to reshape the cornea by removing a thin disc of corneal tissue called a lenticule from a small incision, hence the name Small Incision Lenticule Extraction (Innovations in Vision Correction Surgery, 2023). Additionally, the operation time is short and takes mere minutes for each eye (SMILE Eye Surgery, 2023). To summarize, SMILE is an innovative ophthalmological invention that builds on an existing procedure– LASIK–to make it better.
Lastly, with the rise of artificial intelligence in recent years, AI-powered diagnostics have become one of the most convenient and helpful ophthalmological innovations. By analyzing imaging data, eye conditions and diseases like glaucoma, diabetic retinopathy, and macular degeneration can be detected and diagnosed in their earliest stages, allowing for quicker intervention by optometrists and ophthalmologists and the prevention of further optical degeneration. Additionally, AI is also used to help doctors create treatment plans personalized to each patient, boosting patient care (Top 5 Breakthroughs in Eye Care Technology for 2025, n.d.). Overall, AI-powered diagnostics is a recent innovation that allows for early intervention and better patient care.
In conclusion, these innovations–IOLs, SMILE, and AI-powered diagnostics–have shaped modern ophthalmology as we know it today and are proof of the constant evolution and improvement that the field of ophthalmology undergoes. In regard to the future, we can expect to see more ophthalmological innovations as the next generation of researchers and ophthalmologists emerge.
